Referrals and Guidelines for Referrers

Referrals can be made through your Solicitor, CAFCASS, social worker or you may contact us directly. You can download a copy of our Referrals Form and associated guidelines by clicking on the buttons below. There are two groups of documents, one set for the resident parent, and a second set for the non resident parent. The form comes in two formats, you only need to fill in one of them. There is a PDF version for printing, and an MS Word (Doc) file for editing and saving.  If you are unable to print this form or complete it online please use contact us link and we will send you a copy in the post.

The MS Word version will download to your computer system so you can edit the form locally and send it to us. The PDF version will open in a new browser window. Close the window to return to this page. You can also download the PDF version of the form.

Important: Returning Forms

If you have used MS Word and edited the form on a computer, please save your original locally and use File, Export to save a version in PDF format. You can then send that to us by email, or print it and send it in the post.

If you do not have MS Word on your computer you can download the PDF version of the form and print it. You will need to fill in the form using a black pen. To return the completed form back to us, you can either scan the form and send it back as an attachment, or post it back to us.
